We do not participate with any dental or medical insurance, but that does not mean that we don't provide the service of submitting claims for our patients. We kindly ask that you provide us with all your insurance information, so that we may better assist you in filing and maximizing your benefits.

It is in your best interest to clearly understand your policy; including, but not limited to benefits available, frequencies and limitations. Should you need assistance with this information, we are here to assist you and help you understand your benefits. We will also assist with pre-estimates prior to treatment, allowing you to know what benefits your insurance will be reimbursing you for services proposed.

Insurance is a contract between you and your carrier and not between the carrier and Dr. Samani. You are fully responsible for all fees at the time of service. If your insurance carrier sends us payment, it will be applied to your account. If there is no money due to our office, you will be entitled to a full refund.
